Below, we have provided for you our Official Player Checklist. c 1 Player Registration Form
c 2 Medical Release Waiver Form
c 3 Internet Parental Consent Form
c 4 2010 Season Nonrefundable AAU Basketball Program Individual Player's Registration Fee - $5.00
c 5 2010 Season AAU Basketball Program Player's Fee (Non-refundable after the first game) - Total Payment $250.00
c 6 Uniform Cost - $0.00
c 7 Proof of Age: Clean photocopy of birth certificate, adoption papers, or immigration papers
c 8 Proof of Grade: Clean copy of any report card of the current school year or school document that indicates current grade level.
c 9 "Parental and/or Guardian Support" - Parents are expected to provide transportation to and from practices and tournaments. Players need support from their parents and/or guardians!
c 10 "Sweat" from hard work on the Basketball Court!!!
