The Prosperity Center AAU Basketball Player Registration Form All parent(s) and/or guardian(s) must complete and sign our Player Registration Form for each minor child that intends to play basketball with our organization.

No player will be permitted to participate in any activities (practices, scrimmages, games, special events) without turning in a completed and signed Official Player Registration Form to our administrative staff. There is a $5.00 nonrefundable registration fee which must be paid at the time that this form is turned in to our administrative department. Forms received without the nonrefundable registration fee and a valid signature and date will not be accepted.
