Return Tournament Team Registration Forms, with Certified Checks or Money Orders made payable to "The Prosperity Center" A.S.A.P! 

 

Mail Payments, Registration Forms, and Team Rosters To:

Don't forget to:
  1. include your membership number, if applicable.
  2. indicate division, age/grade of your team.
  3. include your email address and contact information.

 

Important Timeline Guidelines

  1. Make Hotel Reservations.                                                                           20 days prior
  2. Review and familiarize yourself with all Tournament Event Rules.                     7 days prior
  3. Pay balance of entry fees.                                                                          14 days prior
  4. Return Completed Rosters to The Prosperity Center Office                              14 days prior
  5. Distribute Rules/Code of Conduct to players & parents.                                   7 days prior

 

Please Note: Fill out and complete all required tournament paperwork as applicable. Don't forget to 

obtain all required information and signatures for players and coaches before mailing.
